It hooks into the H5_Game.exe process and modifies the DirectX calls in real-time. It forces the game to render at your native resolution (16:9, 16:10, or 21:9) and, crucially, adjusts the Field of View (FOV) . Without the FOV adjustment, you see the "fish-eye" effect on the edges.
