In the writers' room for Season 3, the producers had a vision: Michael would break out of a new prison (Sona in Panama) to avenge Sara’s death. The narrative plan was always for her to die to give Michael a dark, revenge-fueled motivation.
After fan backlash and a successful contract renegotiation, Season 4 famously revealed the truth: The head in the box belonged to an anonymous woman (Sara’s presumed cellmate). Gretchen lied. Sara escaped, went into hiding, and later rescued herself.
In the series finale The Final Break , Sara is wrongfully imprisoned for killing Michael's mother, Christina Scofield, leading Michael to sacrifice his own life so that she can escape.
